Your CU Benefits Express Membership Card

• Keep your membership card with you at all times so it will be easily accessible.  You must present your membership card at the time of service to receive a discount
• Remember all your legal dependents can use your membership card or you may order additional cards for legal dependents with their names
• To locate participating providers in your area, call the appropriate number on the back of your card
• Payment in full must be made at the time of service
• If you have any questions on how to use your membership, call the Member Services department at the toll-free number listed on the back of your card for prompt, friendly assistance
• CU Benefits Express offices are open Monday-Friday 7am - 7pm and Saturday 8am-5pm Central Time.
